The Opportunity:

We are looking for Care Coordinator, Community Health Resource Centre (CHRC) (Part Time) to support the operations team.
 

Job Responsibilities:

 The incumbent shall be responsible to:

  • Perform finger prick Point of Care (POC) glycated haemoglobin (HbA1C) test and blood pressure (BP) for visiting residents
  • Provide individual counselling depending on POC HbA1C test result, blood pressure reading
  • Provide follow up to residents’ post POC HbA1C testing
  • Assist in referring residents to Primary Care Network(PCN) / General Practitioner (GP) or Polyclinic for follow-up
  • Assist visitors in Do It yourself (DIY) Eye check (Snellen Chart), BMI measurement and Body composition analysis where necessary
  • Perform daily equipment readiness check to ensure that it is useable and in good condition
  • Data management, including screening results, questionnaires and forms
  • Inventory management of medical consumables, including ordering of medical consumables
  • Provide general health promotion advice on healthy eating, healthy lifestyle and encourage physical activity, recommendation on vaccination and age appropriate health screening tests
  • Provide diabetes counselling and education when necessary
  • Advise residents on relevant information on health resources and connect them to community health resources.
  • Participation in outreach activities / events in the community
  • Organise and facilitate group activities including health talks at CHRC and around the constituency
  • Any other duties as assigned

Job Requirements:

  • Bachelors' degree or equivalent with 2 – 3 years of relevant work experience in Healthcare and Community Setting
  • Prior experience with Diabetes related counselling will be a bonus
  • Able to work independently and as a team
  • Must be able to work designated centre opening hours and weekends
  • Strong organisational, coordination, interpersonal and communication skills
