This role is based in the polyclinic and you will be part of the multi-disciplinary team involved in the care of patients with chronic conditions, specifically diabetes. You will play a critical role in patients’ care journey and this includes engaging patients in the self-management of their chronic conditions and being their supporter/motivator. You will also be expected to take on some administrative functions to support the implementation team.
Your main duties will include the following:
- Collaborate and cooperate with physicians, nurses and other health care providers in the delivery of patient care and services.
- Preparation of medical reports/letters to be mailed to patients.
- Collection of patient data and maintenance of patient records/databases.
- Perform consent-taking, and administration of surveys/questionnaires.
- Perform accurate documentation in the delivery of patient care.
- Track, recall and reschedule patients who have defaulted their appointments
- Make appropriate referrals to care managers and community partners for interventions according to patients’ needs.
- Engage/communicate with patients on community programmes.
- Provide continuous and relevant updates, and/or submission of reports to reporting officer.
- Precept newly joined coordinators in the delivery of patient care where needed.
- Scheduling, planning of meetings and notes-taking.
- Handle logistics for meetings.
- Comply with guidelines and safe practices according to organizational Work Instructions and Policies.
- Take on additional responsibilities or tasks as assigned by the Reporting Officer.
Requirements:
- NITEC and Diploma, preferably with some working experience in healthcare/community services.
- Proficient in Microsoft Office Applications.
- Savvy with medical systems.
- A good communicator who also possesses tact and excellent interpersonal skills.
- Detail-oriented.
- Able to work independently and as a team.
- Proactive and self-motivated.
